The most common benign neoplasm (tumor) of the epididymis is an adenomatoid tumor. It is a small, benign, slow-growing tumor that typically presents as a painless, firm nodule in the head of the epididymis in young to middle-aged men. It is of mesothelial origin (arising from the lining of body cavities). Histologically, it consists of gland-like spaces and cords of epithelioid and spindle cells. It is entirely benign and cured by simple local excision. Other benign lesions of the epididymis include leiomyomas and cystadenomas (particularly in von Hippel-Lindau disease), but adenomatoid tumor is the most frequent. Differentiating it from malignant tumors or infections is important, and surgery is both diagnostic and therapeutic.
