Reverse grouping, which tests the patient's serum for the presence of anti-A and/or anti-B antibodies, is not reliable in newborns under three months because their immune systems are not yet mature enough to produce these antibodies in detectable levels. Newborns acquire passive immunity from their mothers via placental transfer of IgG antibodies, but these are not the IgM anti-A or anti-B relevant for ABO typing. The infant's own production of 'naturally occurring' ABO antibodies begins around 3-6 months of age after exposure to environmental antigens. Therefore, ABO typing in young infants relies solely on forward grouping (antigen testing) and is considered temporary until confirmatory testing can be done later.
