The primary nursing intervention for a patient with anxiety is providing a calm, supportive environment and using therapeutic communication to help the patient identify and cope with anxiety-provoking thoughts and situations. Nursing actions include: staying with the patient if they are experiencing acute anxiety; speaking in a calm, reassuring tone; encouraging expression of feelings; teaching relaxation techniques (deep breathing, guided imagery); and assisting with problem-solving if anxiety is related to specific concerns. For chronic anxiety, nurses may collaborate with mental health professionals for cognitive-behavioral therapy or medication management. Assessment of anxiety level (mild, moderate, severe, panic) guides intervention. Documentation includes observed behaviors, verbal reports, and effectiveness of interventions. Patient education focuses on recognizing triggers and developing coping strategies. Non-pharmacological approaches are first-line, with medications reserved for severe cases. Reducing anxiety improves overall well-being and adherence to treatment.
