A 'lookback' procedure is a systematic process initiated by a blood collection facility or transfusion service when it is discovered that a **donor** later tests positive for a transfusion-transmissible infection (TTI) such as HIV, HBV, HCV, or HTLV. The purpose is to **identify all prior blood components donated by that individual** during the 'window period' (when they were infectious but tested negative) and to **trace these components to the recipients** who received them. Once identified, the recipients (or their physicians) are notified so they can be tested for the infection and offered appropriate counseling, monitoring, and treatment if necessary. Lookback is a critical component of hemovigilance and post-donation safety, aiming to mitigate the consequences of window period transmissions. It is not about investigating a specific patient's reaction, inventory errors, or donor recruitment.
