This statement is false. As previously noted, prostate cancer has a strong and characteristic predilection for metastasizing to bones, not the liver, as its first distant site. The axial skeleton, particularly the lumbar spine, pelvis, ribs, and femur, is the most common location for metastasis. These bone metastases are typically osteoblastic (bone-forming), appearing as dense, sclerotic lesions on imaging. Hematogenous spread to the liver, lungs, and other organs does occur, but this is generally later in the disease course. The unique tropism for bone is thought to be due to the 'seed and soil' hypothesis, where prostate cancer cells (seed) find a favorable microenvironment in the bone marrow (soil) that supports their growth. Bone pain, especially in the back, is a classic symptom of metastatic prostate cancer.
