Aldosterone increases blood pressure through its action on the kidneys. As a mineralocorticoid hormone, it promotes the reabsorption of sodium ions (Na+) in the distal tubules and collecting ducts. Water follows sodium osmotically, leading to increased water reabsorption and expansion of extracellular fluid volume and blood volume. Increased blood volume raises cardiac output, which, along with aldosterone's potential permissive effects on vascular tone, leads to an increase in blood pressure. Aldosterone also promotes potassium excretion and hydrogen ion secretion. Its secretion is primarily regulated by the renin-angiotensin-aldosterone system (RAAS) in response to low blood pressure, low blood volume, or high potassium. Chronic excess aldosterone, as in primary hyperaldosteronism (Conn's syndrome), is a common cause of secondary hypertension.
