The cornerstone of clinical parasitology is distinguishing pathogenic from non-pathogenic organisms. Chilomastix mesnili fulfills all criteria for a commensal: it resides in the large intestine (cecum and colon), feeds on bacteria and debris, does not invade tissues, and its presence does not correlate with any specific gastrointestinal pathology or symptoms. Numerous studies have failed to establish a causal link between C. mesnili and disease. Therefore, detecting it in a stool sample is considered an incidental finding. Treating it with drugs like metronidazole would be inappropriate, exposing the patient to potential side effects without benefit, contributing to antimicrobial resistance, and, most importantly, diverting attention from the true cause of the patient's symptoms, which could be another parasite, a bacterial infection, or a non-infectious condition like inflammatory bowel disease.
