The clinical presentation (backpacker, watery diarrhea, bloating, foul gas) and microscopic findings (pear-shaped, falling leaf motility) are classic for giardiasis caused by Giardia lamblia. The drug of choice for giardiasis is metronidazole. A typical regimen is 250 mg three times daily for 5-7 days. Tinidazole is a highly effective single-dose alternative. While doxycycline (B) is used for some traveler's diarrhea (e.g., from Campylobacter), it is not effective against Giardia. Ciprofloxacin (C) is for bacterial causes. Mebendazole (D) is an anthelmintic for worms, not protozoa.
