Adenovirus infection in respiratory cytology is characterized by cells with smudged, enlarged nuclei containing large basophilic intranuclear inclusions that often fill the entire nucleus, giving a 'smudge cell' appearance. The inclusions are homogeneous and may push chromatin to the periphery. Infected cells may also show nuclear enlargement and chromatin margination. Adenovirus can cause respiratory illnesses ranging from mild upper respiratory infections to severe pneumonia, particularly in children and immunocompromised patients. The cytopathic effect is distinctive but not pathognomonic, as other viruses can cause similar changes.
