Diphenhydramine has strong anticholinergic (antimuscarinic) effects. In the bladder, parasympathetic (muscarinic) activation contracts the detrusor muscle to promote urination. Blocking these receptors with diphenhydramine relaxes the detrusor muscle, leading to urinary retention, which is especially dangerous in patients with pre-existing BPH.
