Health promotion and disease prevention aim to enhance well-being and reduce illness through proactive interventions at individual, community, and population levels. This includes primary prevention (preventing disease before it occurs - immunizations, health education), secondary prevention (early detection and treatment - screening), and tertiary prevention (managing established disease to prevent complications - rehabilitation). Approaches: behavioral (lifestyle changes); environmental (safe water, clean air); policy (regulations, taxes); and clinical preventive services. Nurses assess risk factors, provide education, conduct screenings, advocate for healthy policies, and empower individuals/communities. The goal is to shift from reactive sick care to proactive health care, addressing social determinants of health. Evaluation measures behavior change, risk reduction, and health outcomes. Documentation includes risk assessments, education provided, and referrals. Health promotion is central to nursing's role in improving population health and reducing healthcare costs.
