The most definitive step in resolving an unresolved ABO discrepancy is to obtain a new, properly identified blood sample from the patient and repeat the entire ABO typing procedure from scratch. This is the gold standard action because many discrepancies arise from pre-analytical errors, such as mislabeling the original tube or drawing blood from the wrong patient. Testing a freshly drawn sample verifies the patient's true blood type and rules out technical errors on the initial sample. The laboratory must not report a final ABO type or release blood for transfusion until the discrepancy is resolved with concordant results from two separate samples or through conclusive specialized testing.
