Preventing heat loss through drying and skin-to-skin contact is crucial for neonatal thermoregulation. Newborns are prone to heat loss due to: large surface area to weight ratio; thin skin with blood vessels close to surface; limited subcutaneous fat; and inability to shiver. The 'warm chain' includes: warm delivery room; immediate drying; skin-to-skin contact; breastfeeding initiation; appropriate clothing/blankets; and keeping mother-baby together. Heat loss mechanisms: convection (drafts); conduction (cold surfaces); radiation (cold walls/windows); and evaporation (wet skin). Signs of cold stress: temperature <36.5°C; lethargy; poor feeding; hypoglycemia; and respiratory distress. Hypothermia increases oxygen consumption and mortality risk. Nurses ensure: pre-warmed blankets/equipment; hats (significant heat loss from head); delay bathing until temperature stable; monitor temperature frequently; and use radiant warmers/incubators when needed. Thermoregulation is vital for metabolic stability.
