NSAIDs inhibit COX-1 in the gastric mucosa, reducing prostaglandin (PGE2) production. Prostaglandins normally stimulate mucus and bicarbonate secretion and maintain mucosal blood flow. Their loss leads to impaired mucosal defense, increased acid penetration, and ulcer formation. NSAIDs also have a direct irritant effect.
