A suicide risk assessment is a systematic evaluation to determine the immediate risk of self-harm and the appropriate level of intervention needed to ensure the patient's safety. It involves asking direct questions about suicidal ideation, intent, plan, means, and past attempts. The purpose is not to predict the future with certainty, but to gauge the current level of risk and use that information to develop a safety plan, which may include increased observation, hospitalization, or connecting the individual with crisis resources.
