The primary nursing intervention for deep vein thrombosis (DVT) is anticoagulation therapy to prevent clot extension and pulmonary embolism while monitoring for bleeding complications. DVT signs: unilateral leg swelling, pain, warmth, redness. Nursing actions: administer anticoagulants (heparin, enoxaparin, warfarin, DOACs) as ordered; monitor for bleeding (gums, urine, stool, bruising); apply compression stockings; encourage leg elevation; avoid massage; and educate about signs of pulmonary embolism (sudden dyspnea, chest pain). Prevention includes: early ambulation; compression devices; anticoagulant prophylaxis for high-risk patients. Nurses monitor coagulation parameters (aPTT for heparin, INR for warfarin). Documentation includes assessment findings, medications, and patient education. Patient teaching: importance of adherence; dietary considerations for warfarin; follow-up monitoring; and signs of complications. DVT treatment reduces risk of fatal PE and post-thrombotic syndrome.
