Bladder washings (also called barbotage) offer several advantages over voided urine for cytological evaluation. They typically yield higher cellularity because the saline irrigation dislodges more cells from the urothelial lining, providing a richer sample. Cellular preservation is superior due to immediate fixation, reducing degenerative changes commonly seen in voided urine. The architecture is better maintained, with cells often present in fragments or clusters that can help assess tissue patterns. Bladder washings are less likely to be contaminated by squamous cells from the distal urethra or vagina, especially in females. They are particularly useful for diagnosing high-grade urothelial carcinoma and for follow-up of bladder cancer patients, though they are invasive and require catheterization.
