Giardia duodenalis (syn. G. lamblia, G. intestinalis) is not a single uniform species but a complex of at least eight distinct genetic assemblages (A-H). This genetic diversity has profound clinical and epidemiological implications. Clinically, certain assemblages (notably Assemblage A and B) are primarily associated with human infection and tend to cause more symptomatic disease. Assemblage B, in particular, has been linked to more prolonged and severe symptoms. Epidemiologically, these assemblages act as markers for transmission pathways. Assemblage A is zoonotic, found in humans, livestock, and wildlife, implicating animal-to-human transmission via contaminated water or food. Assemblage B is more anthroponotic (human-to-human). Identifying the assemblage in an outbreak helps trace the source: is it a water supply contaminated by beaver or livestock feces (Assemblage A), or is it person-to-person spread in a daycare center (Assemblage B)? This knowledge is crucial for designing targeted public health interventions, such as fencing off watersheds from animals versus improving hygiene in institutions.
