Mental status examination (MSE) systematically assesses cognitive, emotional, and behavioral functioning to identify abnormalities and guide treatment in mental health nursing. Components: appearance and behavior; speech; mood and affect; thought process (logic, coherence) and content (delusions, suicidal/homicidal ideation); perception (hallucinations); cognition (orientation, memory, attention, concentration); and insight/judgment. Nurses use MSE to: establish baseline; monitor changes; evaluate treatment response; and assess safety. Documentation should be objective, using quotes when possible. Regular MSE detects deterioration early. Cultural considerations affect interpretation. Mini-Mental State Examination (MMSE) or Montreal Cognitive Assessment (MoCA) screens cognition. Nurses in all settings use abbreviated MSE when mental status changes noted. Training improves reliability. MSE informs communication with treatment team and care planning. It is fundamental skill in psychiatric nursing but useful in general nursing for detecting delirium, dementia, or other changes.
