Color Doppler ultrasound of the scrotum is considered the gold standard imaging modality for evaluating acute scrotal pain. Its primary role in suspected testicular torsion is to assess blood flow. In testicular torsion, the Doppler ultrasound will show absent or significantly reduced blood flow to the affected testicle compared to the normal side. In contrast, in epididymitis or orchitis, there is typically increased blood flow (hyperemia) to the inflamed epididymis and/or testis due to the inflammatory response. Ultrasound also provides excellent anatomical detail, showing the size, echotexture, and any structural abnormalities of the testis and epididymis. While a nuclear medicine testicle scan can also assess blood flow, it is less readily available and has been largely replaced by Doppler ultrasound due to its speed, lack of radiation, and detailed imaging.
