High Fowler's position (sitting upright at 80-90 degrees) optimizes breathing mechanics for patients with respiratory distress by: allowing full diaphragmatic excursion; reducing abdominal pressure on diaphragm; facilitating lung expansion; improving ventilation-perfusion matching; decreasing work of breathing; and promoting secretion drainage. This position utilizes gravity to reduce pulmonary congestion and is particularly beneficial for conditions like: pulmonary edema; COPD exacerbation; pneumonia; asthma; and heart failure. Modifications include: leaning forward on overbed table (tripod position) for COPD patients; semi-Fowler's (45 degrees) for less severe distress; and orthopneic position (sitting upright leaning forward). Nurses should ensure proper support with pillows, bed adjustment, and possibly chairs. Positioning is a simple, cost-free intervention that significantly improves oxygenation and comfort. It should be combined with other measures: oxygen therapy, bronchodilators, diuretics as ordered, and anxiety reduction.
