Finding renal tubular cells in urine cytology may indicate renal tubular injury or disease. These cells are smaller than urothelial cells, cuboidal to columnar, with central round nuclei and granular cytoplasm. They may show reactive changes in acute tubular necrosis, drug toxicity, or renal transplant rejection. While a few renal tubular cells can be normal, increased numbers or atypical forms suggest renal parenchymal pathology. Their identification can be challenging as they may resemble degenerated urothelial cells or lymphocytes. Special stains or clinical correlation may be needed for definitive interpretation.
