Blood products are anticoagulated with citrate, which chelates (binds) calcium. During massive transfusion, a large citrate load can overwhelm the liver's ability to metabolize it, leading to hypocalcemia (low ionized calcium). Hypocalcemia can cause cardiac arrhythmias, hypotension, and impaired coagulation. Calcium replacement (e.g., calcium gluconate) is often given to correct this.
