Factor V Leiden is not a deficiency, but a change (mutation) in the Factor V gene that makes the clotting factor resistant to being turned off. Normally, activated protein C (APC) acts as a natural anticoagulant by inactivating Factor Va. The Factor V Leiden mutation involves a single nucleotide change (a point mutation) at a specific spot in the gene, which replaces the amino acid arginine with glutamine at position 506. This change makes Factor Va resistant to the cleavage and inactivation by activated protein C. This resistance leads to prolonged and excessive thrombin generation, increasing the risk of developing abnormal blood clots, especially deep vein thrombosis.
