Malignant cells arranged in papillary fragments with fibrovascular cores suggest upper urinary tract origin (renal pelvis or ureter), as these structures are characteristic of papillary tumors in the upper tract. While papillary configurations can occur in bladder tumors, well-formed papillae with distinct fibrovascular cores are more typical of upper tract lesions. This finding should raise suspicion for tumors in the renal pelvis or ureter and may guide further diagnostic evaluation such as ureteroscopy or retrograde pyelography.
