The recommended fixative for urine cytology smears intended for Papanicolaou staining is an alcohol-based fixative, commonly a mixture of equal parts 95% ethyl alcohol and ether, or simply 95% ethyl alcohol alone. Alcohol fixation preserves cellular morphology rapidly by denaturing proteins and preventing autolysis. For urine samples, immediate fixation is crucial because urine is hypotonic and can cause cells to swell and lyse. Smears can be fixed by spraying with commercial alcohol-based fixatives or by immersion in 95% alcohol for at least 15 minutes. This ensures optimal nuclear detail and cytoplasmic transparency upon Papanicolaou staining, allowing for accurate assessment of nuclear chromatin, nucleoli, and cellular borders.
