While the exact mechanisms are complex, a key contribution of EBV in NPC is through the expression of the latent membrane protein LMP1 in the malignant epithelial cells. LMP1 acts as a constitutively active mimic of the CD40 receptor, activating multiple signaling pathways. Most notably, it strongly and constitutively activates the NF-κB pathway. This NF-κB activation drives the expression of pro-survival, pro-inflammatory, and anti-apoptotic genes, contributing to the survival, proliferation, and invasive phenotype of the nasopharyngeal epithelial cells, as well as shaping the tumor-promoting inflammatory microenvironment.
