The surgical procedure to repair a varicocele is called a varicocelectomy. The goal is to ligate (tie off) the dilated veins of the pampiniform plexus while preserving the testicular artery, lymphatics, and vas deferens to maintain blood supply and prevent complications like testicular atrophy or hydrocele formation. This can be performed via an open surgical approach (through a small inguinal or subinguinal incision), laparoscopically, or via percutaneous embolization (a non-surgical approach where a radiologist inserts coils or sclerosant into the veins). Indications for repair include pain, testicular atrophy, or infertility with abnormal semen parameters. The surgery often leads to improvement in sperm count, motility, and morphology, and can alleviate discomfort.
