True. Metronidazole is generally contraindicated during the first trimester of pregnancy. While human studies have not conclusively proven it to be a major teratogen (substance that causes birth defects), it has been shown to be mutagenic in bacteria and carcinogenic in rodents at high doses. Due to this potential risk and the principle of precaution, it is avoided in the first trimester when fetal organogenesis occurs. For trichomoniasis or giardiasis in pregnant women, treatment is often deferred until after the first trimester. If treatment is necessary, alternatives like topical clotrimazole (for symptomatic relief of trichomoniasis, not cure) or the non-absorbed drug paromomycin (for giardiasis) may be considered. The benefits and risks must be carefully weighed by a clinician.
