The planning phase of the nursing process is built upon a solid foundation of assessment. Before any plan can be made, the nurse must first systematically gather comprehensive data about the patient. This initial assessment involves collecting information through interview, observation, physical examination, and reviewing records to understand the patient's health status, needs, strengths, and problems. Without a thorough and accurate assessment, any subsequent care plan would be ineffective or potentially harmful. Therefore, assessment is the critical first step that informs every other stage of the nursing process, including diagnosis, planning, implementation, and evaluation.
