The therapeutic effect of antipsychotics on positive symptoms (hallucinations, delusions) is primarily due to blockade of dopamine D2 receptors in the mesolimbic pathway. Blockade in other pathways causes side effects: nigrostriatal (EPS), tuberoinfundibular (hyperprolactinemia), mesocortical (possibly worsening negative/cognitive symptoms).
