Immunocytochemistry is essential to distinguish mesothelioma from adenocarcinoma in pleural fluid cytology. Mesothelioma typically shows positivity for calretinin (nuclear and cytoplasmic), WT1 (nuclear), D2-40 (podoplanin), and CK5/6, while being negative for adenocarcinoma markers like BerEP4, CEA, MOC-31, and TTF-1. A panel approach is recommended as no single marker is perfect. Mesothelioma cells also have a 'window' pattern (clear spaces between cells due to long microvilli) and may show cell-in-cell engulfment. Accurate distinction is critical for prognosis and treatment, as mesothelioma has different management than metastatic adenocarcinoma.
