Balantidium coli invades the mucosa of the large intestine, causing inflammation and ulceration. This leads to a dysenteric illness similar to amoebic dysentery, characterized by frequent, painful stools containing blood and mucus. Other symptoms include abdominal colic, tenesmus (feeling of incomplete defecation), nausea, and weight loss.
