Some receptors have a baseline (constitutive) level of activity even without a natural agonist present. An inverse agonist is a drug that binds to the same receptor as an agonist but produces the opposite effect—it reduces the receptor's constitutive activity below the baseline level. This is different from a neutral antagonist (like Drug Z), which simply blocks the receptor and prevents both agonists and inverse agonists from having an effect, returning activity to baseline.
