The painful, fibrotic condition that can develop as a long-term complication of an episode of priapism, particularly ischemic (low-flow) priapism, is corporal fibrosis. Priapism is a prolonged erection that leads to ischemia (lack of oxygen) within the corpora cavernosa. If the ischemia is severe or prolonged (typically beyond 4-6 hours), it can cause necrosis (death) of the smooth muscle cells and subsequent replacement with non-elastic scar tissue. This fibrosis makes the corporal tissue stiff and inelastic, preventing the normal expansion needed for an erection. The result is permanent erectile dysfunction. The penis may also become deformed or nodular. This is a devastating complication that underscores why priapism is a urological emergency requiring immediate treatment to restore blood flow and prevent this irreversible damage.
