This statement is false. Testicular torsion is infamous for its sudden, severe, and often excruciating onset of unilateral scrotal pain. The pain is typically acute, may wake a patient from sleep, and can be accompanied by nausea and vomiting. The testicle may appear higher in the scrotum (high-riding) and have a transverse orientation (horizontal lie) due to the twisting of the spermatic cord. In contrast, a painless, gradual testicular enlargement is the classic presentation of a testicular tumor, such as a seminoma. This distinction is critical because testicular torsion is a surgical emergency where time is of the essence to save the testicle, whereas a tumor requires urgent but not emergent evaluation. Any acute scrotal pain must be evaluated immediately to rule out torsion.
