Sertoli cells, also known as sustentacular cells, are large, columnar cells that extend from the basement membrane to the lumen of the seminiferous tubules in the testes. Their primary role is to support, nourish, and protect the developing sperm cells (spermatogenesis) through all stages of development. They create a specialized microenvironment called the blood-testis barrier, which isolates developing germ cells from the immune system and provides a controlled ionic and nutritional milieu. Sertoli cells phagocytose residual cytoplasm from maturing sperm and secrete fluid for sperm transport. They also produce inhibin (which provides feedback to the pituitary) and androgen-binding protein (which maintains high local testosterone levels). While Leydig cells produce testosterone, Sertoli cells are essential for translating that hormonal signal into the actual production of sperm.
