A falls risk assessment tool is a standardized screening instrument used to identify patients who are at an increased risk of falling during their hospital stay. By systematically evaluating factors like age, mobility, medication use, cognitive status, and history of falls, nurses can categorize a patient's risk level. The primary purpose is not just to identify risk, but to use that information to implement targeted, individualized preventive interventions (such as bed alarms, non-slip footwear, frequent rounding, or environmental modifications) to enhance patient safety and prevent injury.
