Pancytopenia (low red cells, white cells, and platelets) with a hypercellular bone marrow is a classic presentation of Acute Myeloid Leukemia (AML). The hypercellularity is due to the massive proliferation of the malignant clone of immature cells (blasts). These blasts fill the marrow, crowding out and suppressing normal hematopoiesis, which leads to the low blood counts. According to the World Health Organization (WHO) classification, the presence of 20% or more blasts in the bone marrow (or peripheral blood) is the defining criterion for the diagnosis of AML (with some exceptions like AML with recurrent genetic abnormalities). A bone marrow biopsy is essential for diagnosis, and further classification is based on morphology, immunophenotyping (flow cytometry), and cytogenetics/molecular studies.
