The raw reticulocyte percentage can be misleading, especially in anemia. In severe anemia, even if the bone marrow is responding normally, the percentage of reticulocytes among all red cells will be high simply because the total number of mature red cells is so low. To get a true sense of bone marrow output, we need to 'correct' for this. The corrected reticulocyte count adjusts the raw percentage based on the patient's hematocrit. The Reticulocyte Production Index (RPI) goes a step further, also accounting for the fact that reticulocytes spend extra time maturing in the blood during severe anemia. These corrected values tell you if the bone marrow's response is truly adequate for the degree of anemia, or if it is still sluggish (a hypoproliferative response).
