The milk let-down reflex, or milk ejection reflex, is mediated by the hormone oxytocin. During breastfeeding, suckling by the infant stimulates sensory nerves in the nipple. These signals travel to the hypothalamus, which triggers the posterior pituitary to release oxytocin into the bloodstream. Oxytocin then travels to the breast, where it causes the contraction of myoepithelial cells that surround the milk-producing alveoli. This contraction squeezes milk from the alveoli into the duct system, making it available to the infant. This is a neuroendocrine reflex that can also be triggered by the sound or sight of the baby, or even thinking about feeding. Prolactin is responsible for milk production (lactogenesis), while oxytocin is responsible for its ejection. Without oxytocin, milk production may occur but cannot be effectively released.
