NSAIDs inhibit cyclooxygenase (COX) in the hypothalamus, reducing prostaglandin (especially PGE2) synthesis. PGE2 normally resets the hypothalamic thermostat upward during fever. By blocking its production, NSAIDs allow the thermostat to return to normal, promoting heat loss (vasodilation, sweating).
