Pulmonary cryptococcosis in bronchoalveolar lavage (BAL) shows round to oval yeast cells (5-20 μm) with narrow-based budding and a prominent mucoid capsule that appears as a clear halo around the cell. The capsule stains with mucicarmine (rose-red) or Alcian blue. India ink preparation (not routine in cytology) highlights the capsule as a clear zone. Cryptococcus neoformans is an encapsulated yeast that causes pulmonary infection, often in immunocompromised hosts. The organism may be within macrophages or free. Granulomatous inflammation may be present in immunocompetent individuals.
