NSAIDs inhibit renal prostaglandin (PGE2, PGI2) synthesis. These prostaglandins normally maintain renal blood flow by dilating afferent arterioles, especially in low perfusion states (heart failure, dehydration). Loss of prostaglandins leads to afferent arteriolar constriction, reduced glomerular filtration rate (GFR), and oliguria. They can also cause sodium and water retention.
