Pulmonary hamartoma in fine-needle aspiration (FNA) cytology shows fragments of mature cartilage, adipose tissue, and sometimes bronchial epithelial cells. The cartilage appears as metachromatic fibrillary material on Romanowsky stains. Adipose tissue shows vacuolated fat cells. Epithelial components may include benign bronchial cells. Hamartomas are benign mesenchymal tumors that rarely exfoliate into sputum; FNA or core biopsy is needed for diagnosis. Cytological diagnosis is possible when cartilage and fat are sampled. Distinguishing from other cartilaginous tumors (chondroma, chondrosarcoma) may require imaging correlation.
