A transrectal ultrasound (TRUS)-guided prostate biopsy is the definitive procedure for diagnosing prostate cancer. While an elevated PSA or an abnormal digital rectal exam (DRE) raises suspicion, only a tissue sample examined under a microscope can confirm malignancy. During a TRUS biopsy, an ultrasound probe is inserted into the rectum to visualize the prostate. A spring-loaded needle is then used to take multiple small cores of tissue (usually 10-12, or more in an extended template) from different areas of the prostate, most systematically from the peripheral zone. These tissue cores are sent to a pathologist who examines them for the presence, grade (Gleason score), and extent of cancer cells. This biopsy provides the critical information needed to guide all further treatment decisions. Cystoscopy looks inside the bladder, not the prostate tissue. CT and MRI are used for staging, not primary diagnosis.
