The first-pass effect (or first-pass metabolism) occurs when a drug is absorbed from the gut and passes through the liver via the portal vein before reaching the systemic circulation. If the liver extensively metabolizes the drug on this first pass, a significant portion of the administered dose is destroyed before it can circulate. This reduces the amount of active drug available to the body, which is quantified as a low bioavailability.
