Hydroxychloroquine is an antimalarial drug used as a DMARD in RA. A well-known serious adverse effect is retinopathy, which can present with visual disturbances like blurred vision, scotomas, and difficulty seeing in low light. The macular changes described (bull's eye maculopathy) are classic for hydroxychloroquine toxicity.
