Heparin can cause heparin-induced thrombocytopenia (HIT), an immune-mediated reaction where antibodies form against heparin-platelet factor 4 complexes, leading to platelet activation, thrombosis (e.g., limb ischemia), and thrombocytopenia. The timing (3 days) and findings are classic for HIT.
