Motion sickness is associated with overactivity in the vestibular system and the chemoreceptor trigger zone (CTZ). Diphenhydramine, a first-generation H1 antihistamine, is effective because it blocks histamine H1 receptors and muscarinic acetylcholine receptors in the vestibular nuclei and the vomiting center in the brainstem. This dual blockade reduces nausea and vomiting.
