Nicotine is a ganglionic agonist. It stimulates both sympathetic and parasympathetic ganglia. The net effect on BP is often a rise due to sympathetic outflow. A ganglion blocker prevents all ganglionic transmission, abolishing the effect. A muscarinic blocker removes the opposing parasympathetic effects (like bradycardia), allowing an unopposed sympathetic pressor response, hence a larger BP rise.
