Heinz bodies are intracellular inclusions composed of denatured, precipitated hemoglobin. They form when hemoglobin is exposed to oxidative stress, which causes the globin chains to unfold, aggregate, and bind to the inner surface of the red cell membrane. They are not visible on a standard Wright-Giemsa stain. To see them, a supravital stain (like methyl violet or brilliant cresyl blue) must be used on unfixed cells, where they appear as small, round, purple inclusions usually attached to the cell membrane. Their presence indicates oxidative damage to hemoglobin. This is classic in Glucose-6-Phosphate Dehydrogenase (G6PD) deficiency during a hemolytic crisis, unstable hemoglobinopathies, and exposure to oxidative drugs or toxins. The spleen attempts to remove them, creating 'bite cells' in the process.
